ASSUMPTION OF RISK  AND PHOTO RELEASE AGREEMENT FOR INDEPENDENT GROUPS

By submitting this form, I hereby acknowledge that I have been made aware of the inherent risks involved in On & Off ice sports activities and I knowingly and freely accept those risks. Further, I acknowledge and agree that:

The risk of injury from the activities involved in this On & Off ice sports program are significant, including the potential for permanent paralysis and death, and while particular rules, equipment, and personal discipline may reduce this risk, the risk of serious injury does exist; and I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown of my participation in this independent ON & OFF ice sports group and assume full responsibility for my participation and, I willingly agree to abide by the stated and customary terms and conditions for participation in these types of activities at the ice rink. If, however, I observe any unusual condition or hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest official immediately; and,

I willingly agree to and fully comprehend the risks of COVID-19 by returning to the ice rink. If I observe my player(s) exhibiting any sickness symptoms of any kind I will remove my player from participation and NOT ENTER THE RINK under any circumstances; and, I, for myself and on my behalf of my heirs, assigns, personal representatives and next of kin, hereby release Mass Marauders HC, their officers, officials, agents and/or staff (“Releasees”), with respect to any and all injury, disability, death, medical bill cost, or loss or damage to persons or property, not arising out of the negligence of the Releasees, to the fullest extent permitted by law.

I hereby grant the Mass Marauders HC permission to use my likeness in a photograph, video, or other digital media (“photo”) in any and all of its publications, including web-based publications, without payment or other consideration. I understand and agree that all photos will become the property of the Mass Marauders HC and will not be returned. I hereby irrevocably authorize the Mass Marauders HC to edit, alter, copy, exhibit, publish, or distribute these photos for any lawful purpose. In addition, I waive any right to inspect or approve the finished product wherein my likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of the photo.

I have read and understood the above Assumption of Risk and Photo Release Waiver Agreement and sign it freely and voluntarily without any inducement.
